New Patients
Our office is accepting new patients and most insurance plans are accepted. Please call our office regarding your insurance questions. Insurance verification is recommended prior to your appointment.
Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information and give you time to get to know our staff. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, patient forms and more.
We ask that all new patients arrive a minimum of 30 minutes prior to their scheduled appointment in order to complete the registration process. Patients who have already completed the registration forms (by accessing them through our web site or by picking them up at our office) need only arrive 15 minutes prior to their scheduled appointment time.
Scheduling and Cancellations: Please call our office to schedule an appointment. If you are unable to make your appointment, please notify us 24 hours in advance.
- All new patients are asked to complete the medical history and patient registration forms, and must present their insurance card(s) upon their first visit with our office. It is also the patient's responsibility to bring their referral, if required, with them to our office.
- We ask that all new patients arrive a minimum of 15 minutes prior to their scheduled appointment in order to complete the registration process. Patients who have already completed the registration forms (by accessing them through our web site or by picking them up at our office) need only arrive 10 minutes prior to their scheduled appointment time.
- It is advisable for new patients to bring the most recent labs, imaging studies (actual images on a CD or a hard film along with the Radiologist report if available), list of medications and any relevant medical records to their first appointment.
